Registration Forms & Requirements

HAJJ Visa Requirements (USA)

  *Make one copy of each item and submit it to us with the original copy.
             For example:
          - Your passport & 1 photocopy of the inside picture page;
          - Original signed forms and 1 photocopy of each
            and so on.

NOTE: That only when we receive the HAJJ_SabeelGroup_RegistrationForm and down-payment of $3000 per-person will your seat be initially reserved. If we do not receive full payment & documentation by the set deadline, then there is a chance you will lose your reservation to those on the wait list.

As-Sabeel Registration Form	Write your name exactly as it appears on your passport. Fill out and sign form. (please print legibly)
Saudi Visa Application Form	Fill out and sign form with black ink. We will fill in airline travel information for you once that is confirmed.
Saudi Agreement Form	Read and Sign.
Original Passport	MUST be valid for at least 6 months from departure data and have 3 to 4 empty visa pages, 2 of which are adjacent to each other.
Four(4) Passport Size Photos	MUST be an official passport picture. White background and full-face view (i.e. not tilted to the side). Submit with your full name written on the back of each picture.
Vaccination record for MENINGITIS	Send us the ORIGINAL yellow vaccination card.
Required $274.40 Saudi Moutawaf Agent fee. (Children age 7 to 15 only pay half...$137.20)	You will need to submit to us a CASHIER CHECK* for $274.40 payable to "Unified Agents Office" in Jeddah for pilgrimage services. Without this check the Saudi embassy will reject your hajj visa. *Cashier Checks are attained from your bank. They are NOT Money orders or normal checks.

Notarized Mahram Form	For Women 45years or older traveling alone, you must submit a notarized mahram form. (If you are under 45 you must be traveling with your mahram and submit proof of relation (i.e. brother, son, etc))
Copy of Marriage Certificate for Couples	Copy of Marriage Certificate for Couples to show proof of mahram.
Copy of Birth Certificate for the children
New Muslims Letter of Conversion	New Muslims who have not changed their names on their passpots are required to submit a written statement from their local Islamic center validating their conversion.
Copy of Permanent Resident Card (Green Card)	If not US passport holder you must submit a COPY of your green card (front & back).
Original Permanent Resident Card (Green Card)	If you are a EGYPTION passport holder you must submit your ORIGINAL card.
Certified Letter of Permission from Moroccan Embassy in the USA	If you are a MORROCAN passport holder you must request permission from the Moroccan Embassy in the USA. They will send you a certified permission letter. Submit this form to us.
